About the Role
We are seeking a dedicated Clinical Care Manager (RN) to support our continued growth in Canberra. In this role, you'll lead the clinical coordination and management of services for our home care clients, ensuring exceptional care delivery and a seamless client experience.

Key Responsibilities
Manage intake and onboarding of new clients within Canberra
Conduct comprehensive clinical assessments
Develop, implement, and review individualised care plans
Build strong relationships with clients, families, and external stakeholders
Assist clients in understanding and selecting appropriate care options
Manage Support-At-Home classification within allocated funds
Provide guidance on financial entitlements and responsibilities
Maintain accurate, timely, and compliant documentation
Keep informed of best practices and regulatory changes in home care
Skills, Knowledge & Experience
Essential
Degree in Nursing + at least 1 year experience in a similar role
Current AHPRA registration
Must have a reliable vehicle + valid Australian Driver’s Licence
Must have functional work-from-home setup
Strong clinical assessment and critical-thinking skills
Ability to work autonomously with minimal supervision
Excellent communication and interpersonal skills
Ability to collaborate effectively within multidisciplinary teams
Strong understanding of aged care systems, policies, and regulations
Proficiency with care management software
Strong organisational and time-management skills
Desirable
Understanding of self-management strategies within home care
Familiarity with recent Support-At-Home reforms
