About the Role
The Community Transitions Clinician supports people who experience severe mental illness (and often substance use disorders and/or cognitive impairment) to access the necessary care and support required to ensure a successful transition from custody to the community. The Community Transitions Clinician is part of a multidisciplinary team that works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health and Drug and Alcohol Services and other government and non-government organisations (NGOs) to provide a collaborative approach to ensuring continuity of care. By understanding patients’ clinical, psychosocial and behavioural vulnerabilities, and social and emotional wellbeing needs, the Community Transitions Clinician will utilise general care coordination and discipline specific skills to link patients to relevant services in the community and support their initial reintegration to community care.
The Community Transitions Team is one of the three teams that comprise the NSW Community Forensic Mental Health Services. The team is community based; but provides assertive prison in-reach services to support peoples’ transitions back into the community. Its aim is to develop mental health care plans, establish necessary therapeutic supports for patients, and provide follow-up community care to patients leaving custody. The team is recovery-oriented and promotes positive health outcomes and community participation

The Justice Health Olympic Park (JHOP) office is located at 10 Herb Elliot Avenue, Sydney Olympic Park and Justice Health staff are able to access discounted parking at $5 a day. Public transport is readily available including buses, trains, and ferries. Easy road access is provided via the M4 and Parramatta Road via Homebush Bay Drive. 35 km of bicycle paths surround the site and both bicycle storage and shower amenities are available inside.
