The role focuses on advocacy, care coordination, case support, and community liaison within a residential aged care environment.

Task & responsibilities

• Assessing residents’ social, emotional, cultural, and practical needs and contributing to the development and review of individualised care and support plans
• Interviewing residents and family members to identify difficulties, support requirements, and personal goals
• Monitoring, documenting, and reporting on residents’ progress, wellbeing, and outcomes in accordance with organisational and regulatory requirements
• Referring residents and families to appropriate external services, including health, welfare, housing, disability, and community support agencies
• Liaising with community organisations, welfare agencies, government bodies, and service providers to coordinate services and improve access to resources
• Supporting families, elderly residents, and residents with disabilities through education, advocacy, and emotional guidance
• Assisting residents to address social, emotional, and financial challenges that impact quality of life and community participation
• Promoting awareness of available community resources and services within the residential care setting
• Collaborating with nursing, lifestyle, and allied health teams to support holistic, person‑centred care
• Preparing written reports, case notes, and documentation for internal use, funding purposes, and external agencies as required
• Contributing to service improvement initiatives and community engagement activities aligned with the organisation’s values

Qualification & experience

• Education qualification in Community Services, Social Work, Human Services, Aged Care, or a related discipline
• At least one year experience in community services, aged care, disability services, or residential support environments
• Strong assessment, case coordination, documentation, and advocacy skills
• Ability to work collaboratively with multidisciplinary teams and external agencies
• Knowledge of Australian community support systems, referral pathways, and welfare services
• Excellent communication and interpersonal skills
• Understanding of person‑centred, culturally responsive care practices
