About the Role
As a Community Support Worker, you will:
- Support individuals transitioning into housing environments
- Coordinate assistive technology to promote independence
- Deliver tailored support plans and community education
- Collaborate with allied health professionals and housing providers
- Provide welfare support including referrals, advocacy, and crisis intervention
- Deliver community education and capacity-building programs
- Monitor and evaluate the effectiveness of support strategies and interventions
- Maintain accurate records and ensure compliance with relevant policies
- Assist in budget tracking, procurement, and financial reporting tasks.
What We’re Looking For?
- Diploma or Certificate IV in Community Services, Social Work, Disability, or related field
- 1+ year of Experience in similar role
- Strong communication and interpersonal skills
- Ability to work independently and within a multidisciplinary team
- Knowledge of local housing systems and welfare networks
