About the Role:

We are currently looking for support workers with experience working with participant with high physical disability needs, that includes PEG Feeds and hoisting. You will need experience carrying out PEG feeds, working with hoists and comfortable with giving personal care. You will be required to do short shifts to provide PEG feeds in the morning and the evening, however these hours will increase as we progress. This position requires proven experience working with the above requirements. In addition, you will also be expected to carry out the following in this role:

Applicants must hold valid work rights to work in Australia.

Candidates must hold current and valid NDIS Worker Screening Check and Working with Children Check.

Applicants must have a minimum of 1 year experience working with participants with high physical disability needs.

A minimum of 3 months experience with PEG feeding procedures is required.

Candidates must have at least 3 months experience working with hoisting equipment.

1. Carry out designated duties/tasks as per the service duty roster, and Other reasonable duties as instructed by the employer from time to time

2. To perform personal care duties for individual clients as specified in the personal care profile including (but not limited to):

Ø Showering and bathing

Ø Dressing and grooming

Ø Toileting

Ø Preparing meals

Ø Assisting individual clients with eating their meals

Ø Providing clients with additional support when required, such as gastronomy feeding when appropriate training has been submitted.

Ø Assist with clients physical and mobility requirements in accordance with training provided and the relevant workplace health and safety standard, including transferring of clients.

Ø To provide other support to individual clients as required such as:

Ø Personal administration, shopping for and with clients, banking and paying bills (where applicable)

Ø Supervision of clients medication requirements

Ø Washing and ironing of own clothes

Ø Ensuring that bathing area s and clients appliances are kept in a clean condition

Ø Performing household duties and providing the house is clean and tidy

Ø Tidying of clients rooms

Ø Identifying the need for, and supporting client to attend medical/therapist appointments

Ø Behavioural support

Ø Transport as required

Candidates must have their own vehicle available for transport duties as required.

A current and valid driver's licence is essential for this position.

3. Facilitate community inclusion and participation, dependent on individual needs, abilities and preferences, and support clients to establish and maintain relationships and interests through access to the community activities, venues and services.

4. Carry out as Instructed and/or designated- And demonstrate initiative- Task and activities aimed at meeting the clients personal/living community needs, team objectives and/or program goals which is consistent with Connection Disability services model of practice

5. Carry out and promote positive routines duties activities in the cooperative in respectful and flexible manner, which are meaningful and purposeful to the individual

6. Encourage positive and socially appropriate behaviour and effectively manage challenge behaviour.

7. Promote empowerment and the right choice for all people in need of support within the community

8. Abide by the service communication, reporting and client/program progress practices and procedures, by maintaining appropriate clients records. And work with clients, family members and other stakeholders in a collaborative and supportive manner.

9. Respond to a crisis within specified guidelines (i.e. disability services manual, code of conduct, polices and procedures, service duty roster), and seek assistance to situations outside of specified instructions

10.Participate in developing monitoring individual program plans and be accountable for their outcomes.

11.When required, support and offer assistance to disability support workers (level 2)

12.When needed and/or requested by your supervisor, be involved in on the job training and/or orientation of new staff

13.To be aware of safe working condition and implement secure working practices in all work areas and report potential hazards, incidents and concerns in a professional and timely manner.

14.Maintain current knowledge and understanding of the relevant organisational and departmental policies, procedures and guidelines
To embrace change in line with principles of continuous improvement and play an active role in the future developing and direction of Connection Disability services.
