Join our Community Living Support team in Penrith. We work with people to build independence and confidence by focusing on their values and personal goals, while encouraging social connection and a sense of belonging in the community.
As a Senior Change Facilitator, you will be trained in the Motivational Interviewing change model and will work with clients and stakeholders to assist clients to make significant personal change. You will provide mentorship within the team while assessing individual needs, developing tailored change plans, and delivering flexible services. You will play a key role in building strong relationships, connecting people with community resources, and ensuring high-quality, compliant service delivery through accurate reporting and ongoing review of client progress.
Responsibilities
- Connect and build rapport with individuals from diverse backgrounds, including Aboriginal and Torres Strait Islander communities, CALD and LGBTQIA+ individuals.
- Provide mentorship to colleagues during the referral and development process of co-design personalised support plans and implement strategies to manage risks and promote wellbeing.
- Provide leadership across two designated portfolios, such as, housing, physical health, specific client groups, and work with the team to build capability in these areas
- Monitor, review, and adapt Change Plans in collaboration with clients, mental health services, and stakeholders to support independence and recovery outcomes.
- Deliver and coordinate individual and group support services that address identified needs and foster community participation.
- Build and maintain professional relationships with clients and stakeholders, ensuring confidentiality, cultural sensitivity, and strong professional boundaries.
- Maintain accurate client records and reporting, communicate key updates and risks, and contribute to continuous service improvement and operational efficiency.

Requirements
- Diploma or Certificate IV (Mental Health, Community Sector, Case Management, Counselling or related)
- Current Driver Licence
- Current Working With Children Check
- Current National Police Check
