Outside the Box Support Coordination is a passionate, authentic, dynamic person-centred Hunter Valley based support coordination provider that is privileged to work alongside participants across a range of disabilities, mental health conditions, and complex needs to live their best lives.

This role suits a team player who values flexibility, works well autonomously, and is passionate about empowering NDIS participants to get the most out of their plans. If you have an existing caseload you would like to bring with you, we would love to hear from you.

Employment Type: Permanent Part-time (20-30 hours per week over 3 - 4 days)

Role Responsibilities:

  • Confident, outgoing, bubbly personality, a strong communicator– you're not afraid to pick up the phone or walk into a client’s home with a smile

  • Build and maintain positive relationships with participants, their families and providers

  • Compassionate, organised, and solutions-focused, with a strong commitment to supporting people

  • Interpret and action NDIS plans, identifying funded supports and goals

  • Connect participants with appropriate services and providers with the community and broader systems of support.

  • Monitor participant progress and support plan outcomes

  • Assist with NDIS budget planning and fund management (non-financial)

  • Maintain accurate and up-to-date case notes and reports

  • Attend face-to-face meetings with clients in the Newcastle, Maitland and Lake Macquarie areas

  • Communicate with stakeholders including therapists, providers, and NDIA staff

  • Campaign for the participant's needs and independence alongside mainstream and community services

  • Assistance to resolve points of crisis and developing capacity and resilience in the participant’s network.

  • Coach the participant through challenges that may arise. Helps the participant prepare for review and report on achieved participant outcomes.

 

Role Requirements:

  • A strong understanding of NDIS support categories, and the planning process

  • Excellent communication and time-management skills

  • Ability to work autonomously from home, in community and office

  • Comfortable using email, spreadsheets, and online CRMs

  • Current Driver’s Licence, reliable vehicle and comprehensive insurance

  • NDIS Worker Screening Check, and Working with Children Check or willingness to obtain both

  • An innovative approach, negotiation, and problem-solving abilities

  • Represent the organisation in a professional and positive manner.

  • Adhere to Policies and Procedures and the Code of Conduct.

  • Demonstrate a flexible and team-based work ethic.

  • Attend meetings, training and professional development as required, and to keep skills current to meet the growing demands placed upon the organisation.

  • Perform other duties as delegated and consistent with the position. 

Bonus Points if you have:

  • Previous experience in the NDIS sector (e.g., as a support worker, LAC, or coordinator) or working with Youth and Out of Home Care.

  • Background in community, disability, youth services, out of home care or mental health services

  • Cert IV in Disability, Community Services, or related field
